  • The patient’s name, address, contact telephone number, email address, date of birth. This is so that we can easily contact you with regards to appointment reminders and to pass on to a specialist should you require to be referred for treatment.

  • Patient medical and social background background. This is so that we can provide appropriate treatment.
  • Results of x-rays.
  • Relevant information from people who care for you and know you well such as health professionals and relatives.
  • Financial information for payment of any treatments

We may also share your details with finance companies if you are considering arranging finance for the payment of your treatment or if you have unpaid accounts.We will always obtain your consent to disclose your data before carrying out any referral. Referrals are made through a secure computer network including SCI Gateway and NHS mail accounts.
It is essential that your details are accurate and up to date. Always check that your personal details are correct when you visit us and please inform us of any changes as soon as possible.

Our patients’ personal data is securely stored on-site and only members of staff at Kingsport Dental Clinic have access to our patient records. The patient’s information is only ever accessed as part of their treatment. We only share the patient’s treatment plan with specialist if a referral is required. How long do we keep your information
We conduct regular audits of our data to ensure accuracy and relevance. We generally keep information as long as it is necessary to fulfil our obligations to our patients which under current guidelines is 11 years or to the age of 25 years old, whichever is longer . You have the right to view your records and request to have your information deleted.
